Regarding to lack of BG elements and areas,
I have drawn an area concept art.

This is actually a remake of a scene I had during a nightmare...which is more scary.
However,limited by insufficiency of skills,this is what I can showcase.
I envision the eyes are twinkling and the blood pool is fluctuating but this is just an image so I can't show the dynamics.

Awesome! Now make a monster that lurks in the shadows >:) Maybe make the monster have an effect if it hits you? I was thinking temporary blindness, where the darkness envelopes you can you cant see for a bit.

While I did not specifically drew this for it,I do think this is very suitable for it.
Actually,I have thought about adding it to the image.
But since that is not my intended purpose at the beginning,I did not do so.
